Please complete this form and submit it with a copy of your organization’s Articles of Incorporation OR Certificate of exemption from state sales tax

In addition, please answer the following questions. Unless you are able to answer yes to each of these questions, the organization will be not eligible for grants. If the answer to any of the below questions is no, you may be required to file Form 1023, Application Recognition of Exemption Under Section 501(c)(3) of the Internal Revenue Code with the Internal Revenue Service.
